Registered Nurse – Open Door Project Coordinator

Short Description:

Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre that is committed to and works to improve health outcomes for the most vulnerable immigrants, refugees, and their communities. We do this by facilitating access to services and addressing systemic inequities. The Centre envisions a future in which diverse individuals, families and communities can achieve health with dignity.

Access Alliance is a member of the Mid-West Toronto Ontario Health Team (OHT), and the lead agency for the Open Door Program (ODP), an exciting health initiative aiming to improve health outcomes for systemically vulnerable populations in designated high priority neighbourhoods in our region. ODP activities increase access to health information and services for individuals and communities. The program provides support with preventive care, navigation/referrals to primary care, mental health and addiction services as well as needed community support services.

This is an exciting opportunity to work with the staff team to lead planning, implementation and evaluation of health promotion initiatives such as the Preventive Care Program.  We are looking for a Registered Nurse who is highly motivated and self-directed to be part of our the team. 

Responsibilities:

  • Provide care and support to clients of all ages within the full scope of a RN in specified settings for scheduled appointments, off-site programming and outreach clinics.
  • Initiate and provide care and support to structurally vulnerable clients wishing to access primary health care services and who face multiple and complex health (medical and social) issues.
  • Initiate and implement health promotion strategies such as health promotion and education using community development approaches to support diverse populations and communities.
  • Develop and maintain information resources for the Open Door Program and educational displays on selected health topics as required.
  • Lead the implementation of the Preventive Care Program by participating in planning and overseeing effective delivery of the project activities.
  • Evaluate programs to ensure that objectives are met and recommend changes made to improve practices and systems by applying QI principles and tools.
  • Using a shared leadership, team-based approach, provide guidance and direction to health ambassadors and other project-related staff to achieve identified project outcomes.
  • Build relationships with partner agencies to ensure the success of the coordinated project, and secure client placements or referral sources from the community.
  • Establish and implement service and referral pathways with partner agencies.
  • Develop a workplan in conjunction with the supervisor, identifying actions plans, learning goals and professional development needs.
  • Collect and submit program statistics and reports on a timely basis.
  • Willingness to work evening and weekend shifts when required/scheduled.

Skills and Qualifications:

Required

  • Current registration with the College of Nurses of Ontario
  • BLS certification
  • Bachelor Degree of Nursing

Preferred and/or Demonstrated Experience, Knowledge and Skills

  • 2 or more years of experience in a community and/or team-based care setting;
  • Skill set and experience in: helping clients navigate the health care system and build capacity for self-management, developing and evaluating client education materials,program development, monitoring and evaluation;
  • Working knowledge of: clinical and therapeutic methods, evidence-based practices and health promotion/disease prevention/wellness models;
  • Skill set and experience that supports providing: head to toe clinical assessment, screening, triaging, health promotion and education, working collaboratively with physicians, nurse practitioners and an interdisciplinary health care team as well as working with independence as/when necessary;
  • Demonstrated experience managing projects and budgets
  • Strong health technology and app literacy;
  • Knowledge of and commitment to principles of an anti-oppression and cultural competency framework

  • Ability to communicate effectively in a second language of our priority populations

Salary: 63,337 – 76,394/ Annual 

Duration: One year with possibility of extension

Hours: 35 hours/week
