Short Description:

Access Alliance Multicultural Health and Community Services is a Community Health Centre that provides services and addresses system inequities in order to increase access to primary care and improve health outcomes for vulnerable population that includes immigrants, refugees, and their communities. The Centre envisions a future in which Toronto’s diverse communities achieve health with dignity.

We are looking for a confident, organized, and self-directed Nurse Practitioner (NP) who is calm and solution focused to be part of the Centre’s interdisciplinary clinical team.  The NP will work to full scope of practice within a primary care environment providing episodic care in one of our Access to Care Clinics – “bringing care to where it is needed’’.  The people accessing care in these clinics are newcomers to Canada who have immediate clinical needs, face multiple barriers linked to arrival and settlement and will need connection to ongoing primary care to achieve longer term health and wellness.

Responsibilities:

  • Provision of episodic care to clients of all ages including assessments, diagnosis, screening, referral, education, treatment, counselling and follow up; the focus of this position will be on provision of episodic care; Ability to work with an independent caseload and determine the need for orders, interprets screening and diagnostic laboratory tests, chest and limb x-rays, diagnostic ultrasounds and screening mammography;
  • Monitors client health for specific period of time and, in cooperation with the client and other members of the team, develops appropriate health care plans and recommendations concerning treatment, prevention and health promotion options;
  • Ensures appropriate treatment by maintaining complete and accurate client records with setting priority for ongoing primary care;
  • Participating in primary health care program and service delivery within the broader community context in partnerships with other institutions;
  • Participating in all organizational systems and structures as required including but not limited to, quality improvement and performance evaluation.

Skills and Qualifications:

The ideal candidate will have a firm understanding of and a commitment to the principles and underlying values of community health centers as well as:

Required

  • Current registration as a Nurse Practitioner – Primary Health Care with the College of Nurses of Ontario;
  • Firm knowledge, understanding of and a commitment to the principles and underlying values of community health centers/model of health and well-being;

Preferred and/or Demonstrated Experience, Knowledge and Skills

  • 2 or more years clinical experience as a Nurse Practitioner;
  • Expertise in community or primary health care settings with a focus on immigrant, refugee, non-insured;
  • Expertise in conducting thorough health assessments that include gathering information about medical history, family history, and any pre-existing conditions;
  • Expertise in trauma-informed care;
  • Expertise with chronic disease management and ability to provide education and support to patients and their families;
  • Strong cultural competency skills and ability to recognize and respect cultural differences;
  • Strong interpersonal and communication skills (verbal and written) including active listening and empathy;
  • Superior problem-solving, organizational and time management skills;
  • Thorough familiarity with health promotion/education/prevention models, methods and practices;
  • Ability to work collaboratively within the interdisciplinary health care team which includes physicians, nurses, social workers, dieticians, physiotherapist, health coaches and community workers;

Beneficial Assets

  • Knowledge of immigration policies and laws that affect the population served;
  • Ability to navigate complex healthcare systems to establish access to resources needed by clients;
  • Computer literate (word processing, internet, data entry in Windows applications) and comfort with digital/virtual platforms and service delivery;
  • Knowledge of issues affecting low income, multi-lingual, racialized, and 2SLGBTQI+ communities;
  • An interest and commitment to working in a low-income, multilingual, multiracial community;
  • Ability to speak more than one language.

Salary: 104,476 – 125,880 / Annual 

Duration: 5 Months

Hours: 35 hours/week
