Health Coach and System Navigator – Mental Health

Short Description:

Access Alliance Multicultural Health and Community Services (AAMHCS) is a Community Health Centre that is committed to and works to improve health outcomes for the most vulnerable immigrants, refugees, and their communities. We do this by facilitating access to services and addressing systemic inequities. The Centre envisions a future in which diverse individuals, families and communities can achieve health with dignity.

Our dynamic and collaborative inter-professional team includes physicians, nurses, social workers, dietitians, psychiatric consultants, settlement workers, health promoters, community-based researchers and a cutting-edge health interpretation program. We operate three busy centres, covering the City’s West, East End and Downtown.  Reporting to the Allied Health and Service & Care Pathways Manager, the Health Coach will use a variety of approaches to evaluate need, assess risk, and determine supports required in the context of one on one counselling sessions for clients who experience symptoms of depression, anxiety, post traumatic stress disorder, and social isolation. The Health Coach will assist clients by strengthening constructive coping skills, making connections with mental health services and by helping clients become   self-sufficient in daily living activities.

In addition, the Health Coach will actively participate in health promotion activities, program planning, implementation, evaluation, knowledge transfer, and system change solutions as part of the Allied Health team and other partnerships.

Responsibilities:

  • Provide comprehensive mental health supports to clients within the full professional scope of practice including screening, assessments, counselling, health coaching, referrals, education, support and follow up for scheduled appointments
  • Initiate and implement health promotion strategies such as health education and community development to support diverse populations and communities to increase self-management
  • Working with partner organizations and community groups to identify health priorities and solutions
  • Develop and maintain health information/education resources on relevant topics as required
  • Participate in all organizational systems and structures as required including but not limited to, quality improvement and performance evaluation
  • Ability to travel and provide services throughout the City at designated sites, and to work evening and weekend shifts when required/scheduled

Skills and Qualifications: 

  • You need to be either a social worker/registered nurse/occupational therapists registered with the respective college to be qualified
  • Undergraduate Degree in a relevant major is required, Graduate Degree is considered an asset
  • More than 3 years’ experience in mental health, counselling and community-based work is required
  • Health coaching and systems navigation experience is an asset
  • Demonstrated ability to provide supportive counselling, case management, crisis intervention support and advocacy
  • Experience working in an interdisciplinary team
  • Excellent interpersonal, communication (written and verbal), problem-solving, organizational and time management skills
  • Thorough knowledge of determinants of health and issues affecting low income, multi-lingual, racialized and 2SLGBTQI+ communities
  • Demonstrated ability to work in a fast paced, ever-changing environment

Salary: 63,337 to 76,394/ Annual (Prorated as per the hours of work)

Duration: 12 months with possibility of extension

Hours: Part-time (21 hours/week)
