Description: Access Alliance Multicultural Health and Community Services is a Community Health Centre that aims to improve health outcomes for the most vulnerable immigrants, refugees, and their communities. We do this by facilitating access to services and addressing systemic inequities. The Centre envisions a future in which diverse individuals, families and communities can achieve health with dignity.
We are looking to recruit local community members from Taylor-Massey, Oakridge, or surrounding neighborhoods to lead our Community Health Ambassador (CHA) team. The Community Health Ambassador will play a key role in supporting health promotion strategies to address adverse community health outcomes resulting from the COVID-19 pandemic with Community Health Ambassador Lead and Program Coordinator.
Responsibilities:
- Conduct ongoing outreach activities in Taylor-Massey, Oakridge and surrounding neighborhoods on the topics of cancer screening/prevention, chronic disease management, mental health and food security supports
- Deliver culturally sensitive workshops and outreach materials on the topics of cancer screening/prevention, chronic disease management, mental health and food security supports
- Refer residents of Taylor-Massey, Oakridge and surrounding neighbourhoods to cancer screening, chronic disease management, mental health and food security supports while providing ongoing system navigation care
- Participate in professional development and training opportunities relevant to Community Health Ambassador role
- Participate in regular meetings with Community Health Ambassador team to maintain individual work plan and outreach strategy that responds to current needs
- Provide critical analysis of best practices to Community Health Ambassador Lead
- Foster “peer to peer” community of practice for ongoing information sharing, decompression and issues escalation
- Complete and submit reports at the end of each shift to support Community Health Ambassador Lead in identifying outreach findings and trends
Skills and Qualifications:
- Undergraduate degree in Social Sciences, Community Development or a combination of relevant education and work experience is an asset
- Knowledge of community development and social determinants of health is an asset
- Experience working with newcomers, racialized and/or vulnerable populations, providing front line services and outreach
- Excellent organizational and communication skills
- Ability to understand and respond to changing community needs
- Ability to work independently and collaboratively in a fast-paces environment
- Excellent written and spoken English communication skills
- Multicultural sensitivity and/or experience working in diverse neighborhoods
- Familiarity in working from an anti-oppression, LGBTQ+ positive framework
- Remain agile and ready to adapt to developing program needs
- Ability to speak other languages is an asset: Bengali, Tamil, Arabic, Dari, Tigrinya, Urdu, Pashto
- Ability to travel within Taylor Massey, Oakridge, and surrounding neighborhoods as required
- A minimum of 18 years of age
- A successful clearance of a police vulnerable sector check
Please be advised that our organization requires all staff, students, and volunteers must be fully vaccinated. Proof of vaccination can be obtained from the Ministry site.
NOTE: All staff are expected to work on-site maintaining COVID-19 safety protocols and may be required to travel to and work at other sites as required.
