How to get your COVID-19 certificate in Ontario

  Advance to next screen.
• Open your browser on your phone, tablet or computer and go to covid19.ontariohealth.ca.
  Advance to next screen.
• You are now on the COVID-19 Vaccination Portal page.
  Advance to the next screen.
• Scroll down the page until you reach a checkbox and a statement that says, “I confirm that I read, understood, and accept the full Terms of use.” When you are ready, click on the checkbox. A blue box will appear that says, “Download your enhanced vaccine certificate or book your vaccine appointment“. Click on this blue box.
  Advance to the next screen.
• You are now on the Enter Your Information page. In the first field under “Green Health Card Number“, Enter the 10 numbers on the front of your health card.
  Advance to the next screen.
• In the field called “2-Letter Version Code“, fill in the two letters that appear at the end of the row of ten numbers on the front of your green health card.
  Advance to next screen.
• In the field called “9-character code on the back of the health card“, fill in the code of letters and numbers on the back of your card. 
  Advance to next screen.
• Enter your date of birth.
  Advance to next screen.
• Enter your postal code.
  Advance to next screen.
• Review your information and click on “Continue“.

• You are now on the Choose a Service page.  Click on the “Continue” button beside “Proof of Vaccination“.

• Follow the instructions on the website to download your proof of vaccine.
