Physiotherapist – full time, maternity leave

Access Alliance Multicultural Health and Community Services (AAMHCS) aims to provide services and to address system inequities to improve health outcomes for the most vulnerable immigrants, refugees, and their communities. AAMHCS’s is looking for an experienced and self-directed Physiotherapist to be part of our dynamic and collaborative inter-professional team. This individual is diligent, thoughtful and creative and wants to be part of a team that optimizes the health and well-being of the diverse communities we serve.

RESPONSIBILITIES:

  • Develop care plans by assessing, developing, implementing and evaluating treatments for clients with acute and chronic musculoskeletal or neurological dysfunction, according to the College of Physiotherapists of Ontario guidelines and best practices;
  • Ensure continuum of care is maintained by educating clients and family members, if appropriate, with regard to the management of health care problems and reevaluating physiotherapy programs considering the context of the clients’ home environment;
  • Provide consultation and capacity building to other staff and community agencies working with clients who may need physiotherapy care;
  • Develop, implement, monitor and evaluate, education and health promotion programs for individuals, families and the community, in order to foster self-help, mutual aid, and capacity building in the area of prevention or management of musculoskeletal and neurological problems according to community needs;
  • Assess for assistive devices (mobility equipment) and complete applications, where appropriate, for the Assistive Devices Program;
  • Develop and maintain formal and informal networks within the community to optimize health care service provision and address needs of community;
  • Maintain complete, accurate and timely client records, assessment notes and required correspondence;
  • Actively participate in chart reviews, case conferences, monthly meetings, resource development, quality improvement initiatives and make appropriate internal and external referrals;
  • Actively participate in the Physiotherapy Network of TCLHIN CHCs as directed by manager or designate

SKILLS AND QUALIFICATIONS:

  • Master of Science in Physiotherapy from a recognized university, or an equivalent combination of relevant education and experience;
  • Registered under the College of Physiotherapists of Ontario; Eligibility for membership with the Canadian Physiotherapy Association;
  • Three to five years of experience in a variety of health care settings with experience in community based care an asset;
  • Thorough knowledge of and proficiency in current physiotherapy techniques;
  • Broad range of clinical experience (including chronic disease management) with different populations using a variety of modalities;
  • Demonstrated ability to work effectively in an inter-professional team environment;
  • Demonstrated ability, commitment to and knowledge of community health care;
  • Demonstrated commitment to continuing education;
  • Experience in the use of computers and various software applications;
  • Experience in program development, implementation, monitoring and evaluation;
  • Demonstrated commitment to principles of an anti-oppression and cultural competence framework;
  • Excellent interpersonal, communication (written and verbal), problem-solving, organizational and time management skills;
  • Thorough knowledge of health and issues affecting diverse communities including socio-economic, multi-lingual, racialized and LGBTQ+ communities;
  • ADP authorization (or willingness to obtain) is considered an asset;
  • Second language, relevant to agency service populations, is an asset.
  • Training in manual therapy, acupuncture and/or dry needling considered an asset

Salary: $67,482 – $80,977
Hours: Full-time (35 hours per week)
