Community Health Ambassadors: Vaccination Support Project-West

Access Alliance MHCS is a non-profit, multi-service agency that provides primary health care, social and community outreach services with an emphasis on health promotion to newcomers, immigrants, refugees and their communities in Toronto.

We are looking for local community members from the including the Rockcliffe-Smythe, Mt. Dennis, and Weston communities, to be recruited and trained as Community Health Ambassadors. Community Health Ambassadors (CHAs) will bridge communications between agencies and residents in the community they live. They will play an important part of an integrated engagement strategy to combat the pandemic through COVID-19 health education approaches. CHAs will have the ability to learn, build capacity and connect directly with sources of support, as they will work closely with a network of local service providers. Honorariums will be awarded to all successful applicants.


Community Health Ambassadors will be tasked to meet and inform residents of Rockcliffe-Smythe, Mt. Dennis, and Weston communities. They will connect residents and families in need directly to sources of support, as they will work closely with the local network of service providers.

Community Health Ambassadors are expected to perform the following duties and responsibilities:

  • Provide up-to-date COVID-19 and vaccination information to the residents
  • Attend all training(s) and information sessions hosted by service providers
  • Conduct outreach to promote vaccine clinic information and PPE distribution
  • Provide information and supportive services of Access Alliance and other service providers to ensure optimum delivery of care
  • Connect with residents 2-4 hours a week in designated buildings to encourage vaccination
  • Attending monthly meetings, completing reports and other duties as assigned
    Provide wellness check-in calls to isolated community members
  • Provide regular face-to-face or virtual interactions or group info sessions with residents
  • Refer community inquiries to appropriate community resources and services
  • Provide navigation support to clients, including connecting to services and follow-up
  • Ensures that all communications and records be kept confidential and secure
  • Establish and maintain relationships with landlord, superintendent, building manager(s), tenants, and tenant groups
  • Take notes on community needs, update and maintain accurate information and records
  • Other duties as assigned by project leads

Skills & Qualifications: 

  • Knowledge of community development and social determinants of health an asset
  • Experience working with newcomers, racialized and/or vulnerable populations, providing front line services and outreach
  • Excellent organizational & communication skills
  • Ability to understand and respond to changing community needs
  • Ability to work independently and collaboratively in a fast-paced environment
  • Excellent written and spoken English communication skills
  • Multicultural sensitivity and/or experience working in diverse neighborhoods
  • Familiarity in working from an anti-oppression, LGBTQ+ positive framework
  • Ability to speak other languages an asset: Portuguese, Spanish, Karen, Arabic, Tigrinya, etc.
  • Ability to travel within the Rockcliffe-Smythe, Mt. Dennis, and Weston communities.
  • A minimum age of 18 years of age
  • A successful clearance of a police vulnerable sector check
